Fall + spring Session cancellations

This form is for swimmers enrolled in weekly swim lessons or Swim Skills/Narwhals classes during Fall or Spring Session.

Fill out the form below to cancel a date for your swim lesson or class. Submit a cancellation request for each swimmer and lesson date separately. You will get a confirmation email when your lesson is canceled.

Fitness Alive has a 24-hour cancellation policy:

If you cancel your appointment with at least 24 hours notice, this is an early cancellation and it is eligible for a makeup.
If you cancel your appointment with less than 24 hours notice, this is a late cancellation and it is not eligible for a makeup.
